Participant Waiver & Release of Liability
I understand that participation in athletic training, sports performance activities, turf rentals, gym rentals, strength and
conditioning sessions, agility training, and any related activities at Legacy Sports LLC / Legacy Sports Athletic &
Training involves inherent risks including but not limited to serious bodily injury, permanent disability, paralysis, illness,
property damage, or death. I voluntarily assume all risks associated with participation and facility use. I release and
discharge Legacy Sports LLC / Legacy Sports Athletic & Training, its owners, employees, trainers, contractors,
affiliates, and representatives from any and all liability, claims, demands, actions, or causes of action arising out of or
related to any loss, damage, or injury that may be sustained while participating in activities or using the facilities. I
understand that turf rental and gym rental activities may involve unsupervised usage of equipment and spaces. I
agree to use all facilities responsibly and follow all posted rules and staff instructions. I authorize staff to obtain
emergency medical treatment if necessary and understand that I am financially responsible for any medical expenses
incurred. I acknowledge that photos or videos taken during activities may be used for promotional or marketing
